1. Dangerous Dogs and/or dogs over 25kg in weight
    *    500,000 €  -  175 € per annum
    * 1,000,000 €  -  200 € per annum
2. Non-dangerous Dogs
    *    500,000 €  -  132.50 € per annum
    * 1,000,000 €  -  159.00 € per annum

  1. Full name, postal address, contact telephone number(s) and e-mail of owner.
  2. Name, gender, breed and age of the pet.
